The latest issue of CASE is now available with intriguing reports, including “Severe Mitral Regurgitation and Constrictive Pericarditis in a Patient with Systemic Lupus Erythematosus”, by Jolly et al. CASE Editor-in-Chief, Vincent Sorrell, MD, FASE, remarked, “This month’s highlight is the extremely rare simultaneous cardiac complications in a young man with poorly controlled systemic lupus erythematosus (SLE). Clinical presentations were advanced heart failure leading to hypoxic respiratory failure. In addition to the lupus flare, echocardiography confirmed both severe MR and pericardial constriction. There are impressive TEE findings of a distorted mitral valve with adherent thrombi resulting in thickness up to 18mm and severe MR. This is accompanied by associated echo Doppler and high-quality CMR LGE features of pericardial thickening and hemodynamically significant constraint. For completeness, the authors included the gross and histologic pathology of the replaced valve as well as the pericardium after receiving MV replacement and complete pericardiectomy.”

Accompanying this CASE is an exceptional collection of cases spanning congenital heart disease, autoimmune pathology, structural imaging, and intraoperative guidance. Yao et al. highlight a clinically significant unroofed coronary sinus with multimodality imaging that illustrates its relationship with atrial fibrillation, while Seilani et al. present a rare right atrial appendage aneurysm first diagnosed in adulthood using impressive 2D, 3D, and cross-sectional imaging. Shah et al. describe a compelling pediatric case of complete right ventricular thrombosis following Fontan palliation for Ebstein anomaly. Eimer et al. demonstrate the power of multimodality imaging in identifying a complex pulmonary arteriovenous malformation as the source of embolic stroke. Rounding out the issue, Foster et al. provide a valuable step-by-step TEE guide for transapical aortic cannulation in type A aortic dissection, offering practical insights for imaging specialists involved in the operating room.
